The Power of Connection: How Social Prescribing Supports Healthy Aging

Health is about more than doctor’s appointments and medications. Increasingly, healthcare providers are recognizing that social connection, purpose, physical activity, and community involvement play a vital role in healthy aging.

This presentation introduces the concept of social prescribing—an innovative approach that connects people with community programs and activities that support overall wellbeing. We’ll explore the evidence behind social prescribing, discuss why it matters for older adults, and highlight practical ways to stay active, connected, and engaged in later life. Attendees will leave with a better understanding of how community can be a powerful part of staying healthy.

James Fraser is a Community Health Promoter with Island Health whose work focuses on healthy communities, health promotion, and the social determinants of health. He has worked in mental health and substance use services for nearly a decade and has contributed to research, policy, and community initiatives that support health and wellbeing.

James holds a Master of Arts in Social Dimensions of Health and is completing a Master of Public Administration at the University of Victoria. He is passionate about making public health research practical, accessible, and relevant to everyday life.
