SUMMARY:Palliative Care in the Community: Supporting Quality of Life Close to Home
DESCRIPTION:Palliative care plays an important role in supporting individuals living with serious illness and the people who care for them. This session explores how palliative care can be delivered in community settings through a collaborative\, interdisciplinary approach that focuses on quality of life\, symptom management\, and personalized care. \nParticipants will gain a better understanding of what palliative care is\, when it should be introduced\, and how it can be integrated alongside active treatments from the time of diagnosis onward. The presentation will also highlight the range of community-based supports available\, including home and community care services\, hospice programs\, caregiver resources\, and the importance of advance care planning. \nAttendees will learn how community palliative care helps individuals remain comfortable\, supported\, and connected to what matters most to them\, enabling care that is aligned with their goals and preferences while living at home or in their setting of choice. \nDawn Dompierre\, RN\, is a Palliative Care Nurse Coordinator with Island Health who is passionate about supporting individuals living with serious illness and the families who care for them. With extensive experience in community and palliative care\, Dawn works closely with interdisciplinary teams to help patients access compassionate\, coordinated care and the supports they need throughout their care journey. \nClick here to register.
